Why don’t medical schools teach about the endocannabinoid system (ECS)?

Why are there so few randomized placebo-controlled trials on the benefits of medical hemp and cannabis?

What evidence is there that cannabinoids positively affect human health? Isn’t it all just anecdotal?

The ECS is one of the most extensive neuromodulatory systems in the body with dozens of critical target receptors and its dysfunction has been linked to many chronic conditions

What are some typical conditions that may respond to cannabinoids?

Is there a risk of addiction, schizophrenia or psychosis?

The risk of cannabis addiction in recreational users of THC-dominant cannabis is estimated to be about 9% per the National Institute of Drug Abuse (NIDA), compared to alcohol (10%), cocaine (15%), tobacco (32%), and heroin (23%).
There is no evidence of cannabis addiction with medically supervised use of cannabinoids for medical purposes when minimal effective dosing is employed (Sagar, Kelly A., et al. “Assessing cannabis use disorder in medical cannabis patients: interim analyses from an observational, longitudinal study.” Cannabis 4.2 (2021): 47.)
There is some evidence of increased risk of schizophrenia/psychosis in one particular situation, namely non-medical use of high-dose THC in young teenagers who have genetic predisposition for severe mental illness, especially in a first degree relative (Zammit, Stanley, et al. “Does cannabis use cause schizophrenia? There is no evidence of increased risk of schizophrenia with medically supervised therapeutic cannabinoid use
Cannabidiol (CBD) has well-documented antipsychotic properties and should be included in the regimen if there are any concerns of potential psychosis with THC, for example, in patients with previous serious mental health issues (Iseger, Tabitha A., and Matthijs G. Unsupervised overuse of THC-rich cannabis can not and should not be compared to medically supervised, appropriately dosed cannabinoid use intended for therapeutic outcomes
THC warrants caution due to impairment, but can be easily and safely managed with “start low and go slow” appropriate medical dosing
Non-impairing hemp-derived cannabinoids, such as CBD, CBG, and CBDA, are safe and well-tolerated under medical supervision that includes consideration of cannabinoid-drug interactions
